Registration Information

If you have more than one child participating in athletics you must register each one separately. Registration must be completed for each season (Fall, Winter, Spring).

Athletic Registration
(Grades 6-8)

Registration and completion of forms must be received before the student-athlete participates in any school team-organized practice, scrimmage, or scheduled game/contest.

There is a pay-to-participate fee for athletes in 6th through 12th grades will be $150, with a maximum amount of $600 per family.

Payments for MMS sports may be done by check or electronically by using the  FamilyID system, If paying by check please make checks payable to Region 15. Mail or drop off check to:
